Intensive Case Management, available 24 hours a day, provides assistance to adults with serious and persistent mental illness and to children and adolescents with serious emotional disturbances and their families. The Program provides assistance in gaining access to needed mental health, medical, social, educational and other resources. Activities are designed to provide an individualized approach to serving the daily needs of individuals and families to enable them to develop their strengths and improve the quality of their lives in the community. There are specific Pennsylvania Department of Public Welfare eligibility requirements for the recipients of the service.
Resource Coordination provides case management services to adults and children who have a mental illness and require ongoing assistance in accessing and coordinating mental health and other social services, and assistance in monitoring the provision of those services. Activities are based on an initial assessment of the consumer's strengths and needs obtained from the consumer and, when appropriate, from family members. These activities include assessment service access and monitoring, network building, coordination of services, problem solving and advocacy. There are specific Pennsylvania Department of Public Welfare eligibility requirements for the recipients of this service.
